A Chilean woman fell to her death while skydiving in Brazil on Saturday when both her main and reserve parachutes failed to deploy properly, horrifying footage shows.

Carolina Muñoz Kennedy, 40, was captured on video falling to the ground and wildly losing control.

Passers-by on the ground watched in shock as she fell and eventually hit the street in Boituva, Brazil.

Kennedy had attempted emergency surgery in the fall. She miraculously survived the fall and was treated by paramedics at the scene, but later died from her injuries at a nearby hospital, the Sun reported.

Witness José Soares de Melo described to local media the moment her group found the doomed parachutist.

“She was laying there on the ground waiting for help and people were calling the fire department and the police,” Soares de Melo said.

“I was incredibly sad and shocked.”

After Kennedy's death, police opened an investigation and confiscated her high-performance skydiving equipment for forensic analysis.

It wasn't her first time diving either. Kennedy had taken jumps earlier that day, with the fatal fall set to be the last before it ended Saturday.

Kennedy was registered with the Brazilian Parachuting Association, which will also prepare a report on the causes of the accident.

“During this difficult time, our thoughts are with our dear colleague, her family members and the entire skydiving community,” the BSC told local media.

“Darling, I still can't accept your departure, the girlfriend is so beautiful, intelligent, kind, friendly. I hope that wherever you are you can fly and do your thing with so much passion,” wrote Marisel Karina Pacini Lepe, a friend of Kennedy’s from Chile, on Facebook.

“See you later. Hugs in heaven, friend!”
